  17. disease, or illness certified to be a terminal condition by
  18. two physicians who have personally examined me, one of whom
  19. shall be my attending physician, and the physicians have
  20. determined that my death will occur whether or not life-
  21. sustaining procedures are utilized and where the application
  22. of life-sustaining procedures would serve only to artificially
  23. prolong the dying process, I direct that such procedures be
  24. withheld or withdrawn, and that I be permitted to die
  25. naturally with only the administration of medication, sustenance,
  26. or the performance of any medical procedure deemed necessary to
  27. provide me with comfort care.

  31. regarding the use of such life-sustaining procedures, it is
  32. my intention that this declaration shall be honored by my
  33. family and physicians as the final expression of my legal right
  34. to refuse medical or surgical treatment and accept the
